Please use this form ONLY to notify the office that you will be missing an upcoming session

• Clients must attend an activity at least once every 21 days unless on an approved leave of absence or a fee suspension.

• If you are in the last six months of the 18 month program you cannot miss two consecutive months without attending a make up .

• All missed classes must be made up.

• Make ups can be scheduled by calling the office during business hours at 408 879-7581.
